PRESCHOOL REGISTRATION

Three Options for Registration

1.Print registration packet at www.estill.kyschools.us

2.Pick up a packet outside of the South Irvine ELC lobby doors

3.Request a mailed packet 606-723-4700

Please return the completed packets to:

South Irvine Early Learning Center, 1000 South Irvine Road, Irvine, KY

On Thursdays and Tuesdays from 9:00-Noon-Ring the bell for help.

ELIGIBILITY

To qualify, children must meet one of the following requirements:

• Children who turn four years old by August 1^st and meet income requirements or have a delay/disability
• Children who turn three years old by August 1^st and have a delay/disability or meet Head Start requirements
• Preschool is not a mandatory program; however, our school district is currently exploring options to provide preschool for all children who will turn four years old by August 1^st. If interested in your child attending South Irvine Early Learning Center preschool program, please call for a screening appointment.

ENROLLMENT REQUIREMENTS

• Child’s Certified State Sealed Birth Certificate (not a hospital certificate)
• Proof of Family Income (For eligibility purposes): 1040 Tax Return, W2, SSI Award Letter, Social Security Award Letter, Unemployment Benefit Letter, Employer Letter, Foster Care Letter and KTAP Letter.
• Up-To-Date Immunization Certificate On Kentucky Form Only
• School Entry Physical Exam (with height, weight, blood pressure and lead screening/test)
• Vision Exam by Optometrist or Ophthalmologist
• School Entry Dental Exam (performed by licensed dental professional)
• Medical Insurance Card
• Copy of Legal Guardianship, (if not biological parent)

*The above documents will be required for your child to ATTEND school in August 2020
